Abstract   We tried a phase II study for locally advanced non-small cell lung cancer with a sequential comination of VP-16, cisplatin chemotherapy and radiatherapy. The treatment echedule consisted of 2 cycles of VP 16 (100 mg/m day 1-3), and cispfatin (70 mg/m day 4) given every 3-4 weeks After initial 2 cyles of chemotherapy, patients received radiotherapy consisting of 5,500-6,500 cGy/5.5-6. 5 weeks, and then some patients were on maintenance chemotherapy in non-randomized fashion. This trial was applied to 33 patients during the period of January 1987 to April, 1990. The response rate of initial chemotherapy was 31.0% and that of definitive chemoradiotherapy was 60.0%. Two-year progression-free survivaf of total patients was 19.0%, and median progression free duration was 10.2 manths. Two year progression free survival af responder w as 31.0%, and median progression free duration was 15.0 months. Overall 2-year survival rate was 20.8% and the median survival duration was 13.2 months. Two-year survival of responders was 37.6% and median surival was 18.0 months. The side effects of the treatment were acceptable and reversible. In conclusion, the survival rate using the combination of VP-16, cisplatin chemotherapy and radiotherapy was increased comparing to historical controls. A phase III trial of sequential chemor- adiotherapy versus concomittent chemoradiotherapy is warrented,
